This form is to be completed and approved prior to any Student-Athlete participating in an outside or unattached competition. If there is any additional documentation regarding the Student-Athlete’s participation (e.g., National Team letter, meet entry, etc.), please submit it to the Compliance Office.

By signing this statement, the Student-Athlete confirms that all information provided above is true and accurate to the best of his/her knowledge, and that the provision of false information could render the student-athlete ineligible. Additionally, the student-athlete affirms his/her understanding of, and agrees to abide by, applicable NCAA bylaws, specifically:

- A student-athlete in any sport other than basketball who participates during the academic year as a member of any outside team in any noncollegiate, amateur competition becomes ineligible for intercollegiate competition unless eligibility is restored by the Committee on Student-Athlete Reinstatement. A student-athlete is permitted to practice on such a team.;

- A student-athlete may compete outside the institution's declared playing and practice season as a member of an outside team in any noncollegiate, amateur competition during any official vacation period published in the institution's catalog.;

- For team sports the NCAA limits the number of student-athletes from the same program (with eligibility remaining) that may participate on the same outside team in the following sports:

- In soccer and women's volleyball, a student-athlete may compete outside the institution's declared playing and practice season as a member of an outside team in any noncollegiate, amateur competition, provided:

(a) Such participation occurs not earlier than May 1;

(b) No more than 5 soccer student-athletes and no more than 2 volleyball student-athletes may compete on the same team;

(c) The competition is approved by the institution's director of athletics or designee;

(d) No class time is missed for practice activities or for competition; and

(e) In women's volleyball, all practice and competition is confined to doubles tournaments in outdoor volleyball, either on sand or grass.
